Book your Promo Session

We are excited to begin working with you to help you achieve your health and performance goals. Here are some instructions to help you get signed up and book your first appointment. If you have any questions please feel free to send us an email (holladay.clinic@durohealth) or call (801) 835-6740.

#2 - Enter your promo code

First, make sure you have a promo code.

Second, make sure you are logged into the PTeverywhere application.

Finally, under “Account Settings,” edit the Patient Information, then add the discount code to the last option labeled “ID.” Click the button below for video instructions.

#3 - We will contact you asap

After you enter your promo code, we will verify your information, set up your performance optimization package, and send you a confirmation email.

#4 - Book your first appointment

Once you receive a confirmation email from us, you can book your first appointment. Here is how to do that…

#1 - Log in to the PTeverywhere application.

#2 - Click "schedule."

#3 - Click "Book an Appointment."

#4 - Select the appropriate session, so either "BioneX® Golf Performance Optimization Session" or "BioneX® Ski & Snowboard Performance Optimization Session."

#5 - View the calendar and select an appointment.

#6 - Select "Package/Membership" as payment.

#7 - Select your package and click "apply."
